Record/Preventable?End DateOther NotablesAccident Register Tab:The Accident Register can be used as the required DOT recordable accident register, however, has been designed to record all accidents, regardless of fault and/or severity, and includes a column to identify if the accident was DOT recordable. y|The Accident Register includes filtering capability, as indicated by the drop-down boxes at the bottom right of each column heading. If, for instance, you would like to see only DOT recordable accidents, click on the Recordable? heading. Clicking the drop-down box will provide a list of all entries in that column. Select "Y", and the register will automatically filter, showing only those accidents listed as "Y" for recordable. To reset the register, click the drop-down box again and select "All".`NOTE: If a filter is in affect, the arrow inside the drop-down box will turn from black to blue. .Column headings that include a "Y/N" are yes/no columns only. If anything but Y (for yes) or N (for no) is placed in one of these columns a warning will appear, asking you to change the input. Also, if anything but a Y or N is placed in one of these cells, the Accident Analysis will not work properly.Accident Analysis Tab:The Type column can be used to manually input accident types (i.e. typing "Backing" directly into the cell); however, it was designed to be a tool for Accident Analysis. The Accident Types tab is for classifying common accident types, creating a directory. The corresponding "types" number can then be used in the Type column of the Accident Register, where the Accident Analysis tab will be able to count them by type.lThe Accident Analysis is automatically generated from the information you provide in the Accident Register. Accident Frequency Rates are calculated by mileage; to obtain this information, there is a box at the top for total mileage within the given time frame (same as the time frame for the Accident Register). If you are using the Accident Types directory, the "Accidents by Type" box will display all types currently in use, along with the total number of accidents associated with that type. The most common type of accident will be automatically highlighted.Accident Types Tab:fNOTE: To get the most out of the Accident Analysis, an Accident Types Directory should be established. Create your own Accident Types Directory by placing common accident types in the given table and using their corresponding numbers in the Types column of the Accident Register. We have provided a list of common accident types for your convenience, but any type(s) can be used.IMPORTANT: If using accident type classification numbers, be sure to classify your accidents correctly according to the Accident Types listed in your directory, as it will directly affect your Accident Analysis. Accident Types can be added at any time during the Register's use. If a new type of accident has occurred and does not fit one of the current accident classifications displayed, add it to the next available row.General (all tabs):dAlso take note of any text in red, as it will instruct you as to what should be placed in that cell.Getting started:Select "Save As" (F12 on the top of the keyboard). Name the file to include the year the register will be covering and save in a memorable location.yGo to the Accident Register (see tabs below) and fill in the appropriate cells for Company name, Begin Date and End Date. Begin DateGo to the Accident Types tab and create an "Accident Type Directory" to be used in the "Types" column of the Accident Register. See more notes below..1..2..3.>NOTE: #1 is commonly "your vehicle" and #2 the "other vehicle" The Accident Register is now ready to use. We recommend recording all accidents/incidents, however, if used only as a DOT recordable accident register, the Federal Motor Carrier Safety Administration requires at least the information in the gray columns to be recorded.BEThe Accident Analysis can be evaluated at any time; however, for the most accurate accident frequency rates, you must input your most current mileage estimate.Type (?)Recordable Y/NPreventable Y/NNotes for use:Shaded yellow cells:yare required for the Accident Analysis to work properly. If a shaded yellow cell appears, a value should be placed in it.Notesl [9)8 5!!6""/ # #/ $ $/ & -E1#34,6cc =A dMbP?_*+%&L&D &T&R&P of &N&333333?'333333?(?)?M\\FRONT-PC\Brother MFC-7440NC odXXLetterDINU"TASMTJpBrother MFC-7440NInputBinAUTORESDLLUniresDLLBr_PropsheetDisplayMediaTypeSTANDARDBr_TonersaveOFFBr_DensityOption1Br_ErrorPrintPRINTER_DEFAULTResolutionOption2OrientationPORTRAITPaperSizeLETTERBrPCLMediaType1HalftoneHT_PATSIZE_AUTOBr_RePrintSecureOnlyBr_JobNameOffBr_EndJobNameOffLHUBVirusScannerPSC-FRONT2"@XX??cU} m v} } m} } I} } } } } } "} $ } } $ t,t h@ <@ y z{ wxxxxxxxx ! & & |}~ " # 'I 'I $% "t uL e e e e f f f f Z Z Z [ g PxP&>@<UA 2 7P8:0@ @ Hl !l " B : D ?D l !l " Begin Date : D ?D l !l " End Date ? l !l " Insert Company Name Here: D6 yK Types!A1/ % T w# Yes or No Field;Only Y (for yes) and N (for no) are accepted in this field. % z Putting Time below Date?Hold Alt when pressing Enter to start a new line within a cell._ Numberic Values Only'See "Types" tab for numeric references.w ne gg`Zqqrr @ List121Claim/File No.2 Date/Time3Location (City, St)4Driver5 Unit Numbers6 # of Injuries7# of Fatalities8Towed Vehicle Y/N 9HazMat Spill Y/N 10Citation Issued Y/N 11Recordable Y/N 12Preventable Y/N 13Type (?) 14 Description ;Qrv dMbP?_*+%&333333?'333333?)?M\\FRONT-PC\Brother MFC-7440NC odXXLetterDINU"TASMTJpBrother MFC-7440NInputBinAUTORESDLLUniresDLLBr_PropsheetDisplayMediaTypeSTANDARDBr_TonersaveOFFBr_DensityOption1Br_ErrorPrintPRINTER_DEFAULTResolutionOption2OrientationPORTRAITPaperSizeLETTERBrPCLMediaType1HalftoneHT_PATSIZE_AUTOBr_RePrintSecureOnlyBr_JobNameOffBr_EndJobNameOffLHUBVirusScannerPSC-FRONT2"NXX??cU} } } $ } } %} $} } } $} } } } $} } } I} $ ;K@ w @ ` @@ @@ G C ^D ^I _N 3Z :@ Begin DateB Begin Date ^BG `N:1Z :@End DateB End Date a] F ( (* +I +I ++ ++ H \E <W ;AX 4N BD Not in Use @N/A; @ @@AZBN/A 2_ 5N IZ[Empty] Z B%@ Not in Use:B Not in Use =>> ?N ?> OO >>@X 6N BD Not in Use @N/A; @ @@AZBN/A 3_ 7N IZ[Empty] Z B%@ Not in Use:B Not in Use AP ))5 B ;@@B;B B- T .N#>D Input MileageD .ADB Input Mileage M X 6N BD Not in Use @N/A; @ @@AZBN/A 3_ 7N IZ[Empty] Z B%@ Not in Use:B Not in Use CK //( 0 ; @ @YAZ 0/ T 14N >D Input MileageD .ADB Input Mileage M X 6PNhoBD Not in Use @N/A; @ @@AZBN/A 3_ 7XN IZ[Empty] Z B%@ Not in Use:B Not in Use CJ //( 0 ; @ @YAZ 0/ T 1pN >D Input MileageD .ADB Input Mileage M X 6N BD Not in Use @N/A; @ @@AZBN/A 3_ 7NIZ[Empty] Z B%@ Not in Use:B Not in Use C //> 0 (; Claim/File No.%A 0/ T DN >D Input MileageD .ADB Input Mileage M X6N BD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseEFFFFGHFFIX6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in Use X6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ [Empty] Z B%@ Not in Use: B Not in UsePX6(NBD Not in Use @N/A; @ @@ AZBN/A3_70NIZ [Empty] Z B%@ Not in Use: B Not in UsePX6HNBD Not in Use @N/A; @ @@ AZBN/A3_7PNIZ [Empty] Z B%@ Not in Use: B Not in Use SX6hNBD Not in Use @N/A; @ @@ AZBN/A3_7pNIZ [Empty] Z B%@ Not in Use: B Not in UseSX6NBD Not in Use @N/A; @ @@ AZBN/A3_7NIZ [Empty] Z B%@ Not in Use: B Not in Use XQYYY)U))VX6NBD Not in Use @N/A; @ @@ AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseT QRQ) - ;@@>0AZ ) QV Q( - ;@@YAZVX6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseT RSR) / ;@@>0AZ ) RU R( / ; @ @YAZVX6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseT RTR( / ;@@YAZ ))))VX6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseEFFFFFFFWX6(NBD Not in Use @N/A; @ @@AZBN/A3_70NIZ[Empty] Z B%@ Not in Use:B Not in Use X6HNBD Not in Use @N/A; @ @@AZBN/A3_7PN IZ[Empty] Z B%@ Not in Use:B Not in UseX6hNBD Not in Use @N/A; @ @@AZBN/A3_7pNIZ[Empty] Z B%@ Not in Use:B Not in UseX6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in Use X6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in Use X6NBD Not in Use @N/A; @ @@AZBN/A3_7NIZ[Empty] Z B%@ Not in Use:B Not in UseX6NBD Not in Use @N/A; @ @@AZBN/A3_7N IZ[Empty] Z B%@ Not in Use:B Not in UseD l&<&FH.
